Refund Policy of Summer Camp fee 夏令营退费政策
For 4 weeks or Session 1 only 两期连报或仅报名第一期：
If cancellation is before and on 29 May, full refund 5月29日或之前取消，全额退费；
If cancellation is before and on 5 June, 75% refund 6月5日或之前取消，退 75%；
If cancellation is before and on 12 June, 50% refund 6月12日或之前取消，退 50%；
If cancellation is before and on 19 June, 25% refund 6月19日或之前取消，退 25%；
If cancellation is after 19 June, no refund 6月19日之后取消，费用不退。
For Session 2 only 报名第二期:
If cancellation is before and on 12 June, full refund 6月12日或之前取消，全额退费；
If cancellation is before and on 19 June, 75% refund 6月19日或之前取消，退 75%；
If cancellation is before and on 26 June, 50% refund 6月26日或之前取消，退 50%；
If cancellation is before and on 3 July, 25% refund 7月3日或之前取消，退 25%；
If cancellation is after 3 July, no refund 7月3日之后取消，费用不退。

如果发生意外伤害或严重疾病，我授权给上海耀中带我的孩子前往医疗/齿科诊所就医。这个授权必须在学校通过合理方式联络我却无法取得联系的紧急情况下才生效。我理解我有责任向学校办公室提供最新的紧急联络信息，当我出差或因其它原因无法联系时向学校提供其它联系方式。我确认我已经向上海耀中提供了这些信息并确保一旦联络信息发生变化，我会及时告知办公室。In the event of accidental injury, or serious illness, I authorize YCIS to take my child to the medical / dental clinic to be assessed. This will be carried out in case of emergency or in the event that I cannot be contacted by YCIS, by reasonable means. I understand that it is my responsibility to keep the school office informed of my most up to date contact details for use in case of emergency, including providing alternative contact information for times when I may be travelling or otherwise unavailable. I confirm that I have provided YCIS with these details and that I will update the office should any contact information
我理解，如果我的孩子在夏季活动营期间意外受伤或罹患疾病，上海耀中和夏季活动营提供人不会为此承担任何责任。本人特此放弃就上述意外伤害或疾病向上海耀中和/或其任何员工追究法律责任的权利，除非此类伤害或疾病是由上海耀中员工和/或活动营提供人的重大过失或故意不当行为造成的。I understand that YCIS and the Camp Provider staff will not be held accountable or liable for expenses incurred for my child if he/she suffers an accidental injury or illness during the Summer Camp. I hereby waive my rights to pursue legal liability against YCIS and/or any of its staff in connection with aforesaid accidental injury or illness, unless such injury or illness resulted from the gross negligence or intentional misconduct of YCIS staff and/or any of its Camp Provider staff.
